HOW TO ENTER: Enter by filling out an online registration form and submitting to
Machalek Communications, Inc. via the Web, or by filing out an entry card found in a Machalek Communications Postcard Deck. Limit one entry per person.
RANDOM DRAWING: Six random drawings will be held throughout 2008 among all eligible entries received. The names of winners will be subject to verification. The winner will be notified via email. Decisions of judges are final and binding. Odds of winning depend upon the total number of eligible entries received.
ELIGIBILITY: Eligible recipients of postcard decks published by Machalek Communications, Inc.. Winner must be employee of the veterinary hospital registered on the drawing form at the time the drawing takes place. Employees of Machalek Communications, Inc., their affiliates, distributors, subsidiaries, advertising and promotion agencies and their immediate family members and/or those living in the same household of each are not eligible.
PRIZE AND APPROXIMATE RETAIL VALUE: (6) Grand prizes will be awarded throughout 2008 – 2007 Apple iPod Touch™. Manufacturer’s warranty will transfer to winner; however, iPod Touch™ is awarded “as is.” The Apple iPod Touch™ will be delivered to winners’ business address or other mutually agreeable location. (8GB iPod Touch™: $299).
CONDITIONS OF PARTICIPATION: void in Puerto Rico and where prohibited by law. All federal, state and local taxes are the sole responsibility of the winner. All federal, state and local laws and regulations apply. All materials submitted become the sole property of sponsor and will not be returned. Potential winner must sign and return an Affidavit of Eligibility and Liability/Publicity Release prior to accepting prize. Noncompliance within the time period specified in the affidavit will result in disqualification and an alternate will be selected. In the event Sponsor determines that the winner is ineligible to receive the prize for any reason, Sponsor reserves the right to conduct a random drawing from among all eligible entries received after the drawing has been conducted. No substitution or transfer of prize permitted except that Sponsor reserves the right to substitute prize of equal or greater value in the event that an offered prize is unavailable. Acceptance of prize constitutes permission to the sponsor and its agencies to use winner’s name and/or likeness for purposes of advertising and trade without further compensation unless prohibited by law. By accepting prize, winner agrees to hold Sponsor, its respective directors, officers, employees and assigns harmless against any and all claims and liability arising out of use of prize. Winner assumes all liability for any injury or damage caused or claimed to be caused, by participation in this promotion or use or redemption of any prize. By participating in this promotion, entrants agree to be bound by the Official Rules and the decisions of the judges. Sponsor is not responsible for any typographical or other error in the printing of the offer, administration of the drawing or in the announcement of the prize.
Sponsor is not responsible for lost, delayed or misdirected entries or for entries not received in time for the random drawing. Altered, mutilated or illegible entries will be disqualified.
The iPod Touch™ prize in this promotion is defined as the 2007 iPod Touch™ model. No accessories or services are promised or intended to be a part of the iPod Touch™ prize award.
